PCN Métis Patient Navigator
JOB OVERVIEW
NIMA is looking for a full-time Métis Patient Navigator (MPN) to support Métis community through the complexities of the BC health care system. Reporting to the Executive Director of NIMA, the purpose of this position is to ensure that Métis community members are able to find information needed for their health concerns and connect them with the appropriate health care providers and programs. The MPN will collaborate with Métis Nation BC, Island Health, FNHA, Campbell River and District Division of Family Practice, Campbell River Primary Care Network (PCN) health clinics and staff, local First Nations, and local Indigenous organizations to support and facilitate safe and accessible primary care and community-based health care services for Métis people on northern Vancouver Island. The MPN's mandate is to assist Métis community members with access to health services. As a personal guide, the MPN is there to help the patient and family through their health care journey both in hospital and community. The MPN acts as a bridge between the Métis community and the health care system, through consultation and referral services.
The position is full-time and starts as soon as possible. The salary range is $64,350 - $85,800 per year (depending on experience and education) and includes extended health and dental benefits.

North Island Métis is one of the partners of the Campbell River Primary Care Network (PCN). A PCN is a geographic space where all community-based primary healthcare providers are networked together. One of the goals of PCNs is to enhance patient care using a team-based care approach. The implementation and delivery of the PCN is centred on culturally informed care and cultural safety and humility. PCN positions are part of a cohesive team where each provider has their own unique role and scope in the provision of patient care, as you work together to make sure that the patient’s needs are met in the most effective way possible.

RESPONSIBILITIES
- Assist Métis community members in accessing health services, supports and programs
- Regularly connect with Métis community members to support community growth and healing
- Liaise with local health care facilities and staff to support Métis community members accessing services
- Collaborate with local health care facilities and staff to ensure best practice regarding Métis health
- Assist patients with completing paperwork related to their health care needs
- Develop culturally appropriate resource materials, including local health services, programs and emergency contact numbers
- Be a positive community role model, working in ways which empower and build capacity
QUALIFICATIONS
Education, Training and Experience
- Post-secondary degree in a health-related discipline preferred, but some combination of education, Métis cultural knowledge and/or relevant experience is satisfactory
- Experience in promoting best practices as it concerns Métis or Indigenous Peoples is preferred
- Additional training such as first aid, non-violent crisis intervention, etc. is an asset but not required
- Preference will be given to candidates of Métis ancestry
Knowledge, Skills and Abilities
The ideal candidate will have some or all of the following:
- Familiarity with Métis health challenges
- Knowledge of Métis specific health care supports and resources
- Have a broad knowledge of local health care system and facilities
- Ability to work respectfully amongst many levels of hierarchy and administrations
- Ability to work collaboratively and effectively with other staff as well as other inter-disciplinary partners in both the Indigenous and non-Indigenous environments
- Strong time management and organizational skills, including keeping up-to-date records and providing timely reports
- Ability to deal effectively with conflict and apply dispute resolution and problem solving and negotiating skills in an effective manner
- Proficient computer skills with Microsoft Office; navigating and accessing relevant online information (e.g., government forms)
- A valid BC driver’s license and reliable vehicle
ADDITIONAL NOTES
- Candidates must be willing to work flexible hours – evening and weekend work may be required
- NIMA requires all employees to be fully vaccinated against COVID-19. Candidates must provide Proof of Vaccination
- Must provide a current Criminal Record Check with the Vulnerable Sector
- Pursuant to section 41 of the BC Human Rights Code, preference will be given to applicants who are Métis, or Indigenous (First Nation, Inuit)
